太極拳鍛煉周期對(duì)原發(fā)性高血壓的血壓及心血管危險(xiǎn)因素的影響:系統(tǒng)性評(píng)價(jià)和Meta分析

摘要:

目的 采用Meta分析系統(tǒng)性評(píng)價(jià)太極拳鍛煉持續(xù)周期是否影響原發(fā)性高血壓患者的治療效果,為原發(fā)性高血壓患者提供合理的運(yùn)動(dòng)干預(yù)方案。方法 檢索從中國(guó)知網(wǎng)、萬方數(shù)據(jù)庫、PubMed、Cochrane Library、Science Direct、Web Of Science等電子數(shù)庫中關(guān)于太極拳對(duì)原發(fā)性高血壓治療效果的隨機(jī)對(duì)照試驗(yàn),采用RevMan 5.3軟件進(jìn)行Meta分析。結(jié)果 最終篩選21篇文獻(xiàn),總計(jì)有2 129名參與者。Meta分析結(jié)果提示:與對(duì)照組相比太極拳鍛煉持續(xù)周期12周以上更有利于收縮壓[MD=-10.20,95% CI(-14.57,-5.83)]和舒張壓[MD=-5.12,95% CI(-7.63,-2.60)]水平的降低;能提高原發(fā)性高血壓患者NO含量[MD=6.87,95% CI(4.83,8.90)];改善高血壓患者除HDL-C之外的血脂代謝水平:TC[MD=-0.51,95% CI(-0.69,-0.33)]、TG[MD=-0.72,95% CI(-1.09,-0.36)]、LDL-C[MD=-0.59,95% CI(-0.87,-0.32)]。亞組分析結(jié)果提示運(yùn)動(dòng)頻率≥5次/周、運(yùn)動(dòng)時(shí)間<60 min/d有較好的降壓效果。結(jié)論 Meta分析表明,與其他持續(xù)周期相比,太極拳鍛煉持續(xù)周期12周以上,每次運(yùn)動(dòng)時(shí)長(zhǎng)控制在60 min以內(nèi),每周運(yùn)動(dòng)5次以上對(duì)降低血壓水平、提高NO含量,改善血脂代謝效果最為明顯,應(yīng)值得在臨床上推廣。

Abstract:

Objective To provide a reasonable exercise intervention program for patients with essential hypertension(EH) by systematically reviewing, the aiming is to assess if the continuous cycle of Tai Chi Chuan exercise impacts the therapeutic effects on EH patients by the meta-analysis. Methods The randomized controlled trials(RCT) of Tai Chi Chuan treating EH were retrieved from electronic databases involving CNKI,WANFANG Database,PubMed,Cochrane Library,Science Direct and Web Of Science,and the meta-analysis was conducted by RevMan5.3 software. Results Twenty-one papers with 2 129 participants were finally selected. Meta-analysis results showed that Tai Chi Chuan exercise lasting over 12 weeks was more conductive to the decreased level of systolic blood pressure [MD=-10.20,95% CI(-14.57,-5.83)] and diastolic blood pressure [MD=5.12,95% CI(-7.63,-2.60)] compared with the control group. Tai Chi Chuan was found to increase the NO content of EH patients [MD=6.87,95% CI(4.83,8.90)], improve blood lipid metabolism level including total cholesterol (TC) [MD=0.51, 95% CI(-0.69,-0.33)], triglyceride (TG)[MD=0.72,95% CI(-1.09,-0.36)],low-density lipoprotein cholesterol (LDL-C) [MD=0.59,95% CI(-0.87,-0.32)] excluding high-density lipoprotein cholesterol (HDL-C). The results of subgroup analysis showed that exercise frequency ≥5 times/week and exercise duration< 60 min/d had better antihypertensive effect. Conclusion Meta-analysis indicate that Tai Chi Chuan exercise with the continuous cycle>12 weeks, the frequency ≥5 times/week and exercise duration< 60min/d demonstrates obvious effect in reducing blood pressure level and improving NO content and lipid metabolism, being worthy of promotion in clinic practices.
