Alterations of mean diffusivity of pedunculopontine nucleus pathway in Parkinson's disease patients with freezing of gait.
Abstract
BACKGROUND:
Although freezing of gait (FOG) is a common and disabling symptom in Parkinson's disease (PD), the underlying mechanism of FOG has not been clearly elucidated. Using analysis of diffusion tensor imaging (DTI), we investigated anatomic structures associated with FOG in PD patients.
METHODS:
We enrolled 33 controls and 42 PD patients (19 patients with FOG and 23 without FOG). DTI data were compared between PD patients and controls, and also between PD patients with and without FOG. Whole brain voxel-based analysis and regions of interest analysis in the pedunculopontine nucleus were used for DTI analysis.
RESULTS:
Compared with normal controls, PD patients showed microstructural changes in various subcortical structures (substantia nigra, globus pallidum and thalamus), frontal and insula cortex. PD patients with FOG demonstrated altered mean diffusivities in subcortical structures connected with pedunculopontine nucleus, such as basal ganglia, thalamus and cerebellum in voxel-based analysis. Using region of interest analysis of pedunculopontine nucleus, fractional anisotropy values were reduced and mean diffusivity values were increased bilaterally in PD patients with FOG. In correlation analysis, the fractional anisotropy value of the right pedunculopontine nucleus was moderately correlated with the severity of FOG.
CONCLUSIONS:
Based on our results, microstructural changes of pedunculopontine nucleus and connected subcortical structures are closely related with FOG in PD patients.

盡管凍結(jié)步態(tài)(FOG)是帕金森病(PD)中常見(jiàn)的致殘性癥狀,但FOG的潛在機(jī)制尚未明確闡明。我們采用擴(kuò)散張量成像(DTI)分析來(lái)研究PD患者中與FOG相關(guān)的解剖結(jié)構(gòu)。
方法:
我們招募了33名對(duì)照組和42名PD患者(19名為FOG的患者,23名患者無(wú)FOG)。比較PD患者和對(duì)照之間的DTI數(shù)據(jù),以及FOG(+)與FOG(-)的PD患者之間的DTI數(shù)據(jù)。采用基于體素的全腦分析和感興趣區(qū)分析對(duì)腦橋神經(jīng)核進(jìn)行DTI分析。
結(jié)果:
與正常對(duì)照組相比,PD患者在不同皮層下部位(黑質(zhì),蒼白球和丘腦),額葉和島葉皮質(zhì)中顯示出微結(jié)構(gòu)改變。在基于體素的分析中,F(xiàn)OG(+)PD患者在與橋神經(jīng)核連接的皮層下部位(例如基底神經(jīng)節(jié),丘腦和小腦)表現(xiàn)出平均擴(kuò)散速率的改變。在感興趣區(qū)分析中,F(xiàn)OG(+)PD患者腦橋神經(jīng)核表現(xiàn)出其雙側(cè)各向異性分?jǐn)?shù)值減少和平均擴(kuò)散值增加。在相關(guān)分析中,右側(cè)腦橋神經(jīng)核的各向異性分?jǐn)?shù)值與FOG的嚴(yán)重程度呈中等相關(guān)。
結(jié)論:
基于我們的結(jié)果,腦橋神經(jīng)核和連接的皮層下結(jié)構(gòu)的微觀結(jié)構(gòu)變化與PD患者的FOG密切相關(guān)。
